We need to have on file a record of your child's immunizations. If you haven't already done so, would you please obtain copies of all immunizations your child has received from either your physician or health care provider and send it to us so we can update our records.

Dear Parents: Please help us keep your children healthy. If they have a temperature of 100 degrees or higher, please keep them home. Please call the absentee line if your child is going to be absent for the day.

Please send a copy of your child's most recent physical examination to the clinic so that we can update our records.
